Social Services Specialist for MSS

Sea Mar Community Health Centers, a Federally Qualified Health Center (FQHC), seeks a dedicated Social Services Specialist to join our Maternity Support Services Program at our Salmon Creek Medical Clinic.
The ideal candidate will possess a strong understanding of patient-centered care and be able to work effectively with an interdisciplinary care team. Key responsibilities will include conducting initial screenings, developing comprehensive integrated clinical psychosocial assessments, and providing brief treatment plans.
Additionally, the selected candidate will be responsible for providing transition services for long-term counseling/treatment and infant case management. A strong background in working with underserved populations, including those with limited English proficiency, is essential.
Key Qualifications:
- Knowledge of the Patient-Centered Health Home Model of care
- Ability to work effectively with an interdisciplinary care team
- Strong listening, verbal, and written communication skills
- Knowledge of medical terminology and electronic health record platforms
- Bachelor's degree from an accredited university and current Certified Counselor or Chemical Dependency Counselor certification
Certifications, Licenses, and Registrations:
- Washington State driver's license
- Background check from Washington State Patrol
- Basic Life Support (BLS) CPR certification
