  • Oversees patient scheduling, adjusting appointments as necessary for staff absences, and ensuring timely reassignment of patients.
  • Initiates necessary infection control documentation and submits completed reports to the HR department for record-keeping.
  • Acts as a backup during peak hours, managing incoming calls from field staff and aiding in weekly case discussions, while directing clinical inquiries to the Branch Director as needed.
  • Maintains a log of client hospitalizations, documenting coordination notes and distributing electronic updates to all relevant office, field, and sales personnel.
  • Completes scheduling tasks as they appear on the action screen, ensuring skilled nursing and injection visits are appropriately arranged.
  • Handles scheduling for additional visits and necessary orders, including:
    • Arranging discharge or OASIS collection visits post-case conference.
    • Scheduling TIF OASIS collection visits and updating the remaining schedule.
    • Rescheduling visits that were declined or missed, if applicable.
    • Processing reassigned and rescheduled appointments.
    • Ensuring supervisory visits are properly scheduled.
  • Generates all relevant scheduling reports, including Agent Summary Reports and Missed Visits Reports.
  • Prepares weekly staff schedules, reviewing for productivity and geographic considerations before submitting for Branch Director approval.
  • Verifies visit documentation in the scheduling console as required.
  • Facilitates the internal transfer of patients between different branch offices.
  • If applicable, reviews lab reports for normality, forwarding copies to the physician and Case Manager, while also routing to the Medical Records Department.
  • If clinical duties are required, may perform patient visits and participate in on-call rotations.

Qualifications:

  • Must be a Licensed Professional Nurse or Licensed Vocational Nurse in the practicing state.
  • Minimum of one year of experience in home health care.
  • Prior experience with packet reviews or quality improvement is preferred.
  • Coding certification is a plus.
  • Must have a valid state driver's license and automobile liability insurance.
  • Current licensure in the state of employment is required.
  • Excellent communication skills, the ability to engage with a diverse population, strong organizational capabilities, and the ability to prioritize multiple tasks are essential.

Work Schedule
40 hours per week

Compensation
The salary range provided reflects a good faith estimate of the starting base pay for full-time employment. Actual compensation may vary based on geographic location and individual qualifications, including skills, experience, education, and certifications. The range is $37,200 - $51,200 annually.
