Join Our Team as a Mental Health Case Manager

We are seeking a dedicated and compassionate Mental Health Case Manager to join our team at Mental Health Resources. As a key member of our team, you will play a vital role in providing support and services to individuals recovering from mental illness.

About the Role

The Mental Health Case Manager will work closely with clients to develop and implement individualized plans to promote recovery and stability. This will involve coordinating services and supports, providing education and collaboration with significant others and family members, and ensuring that services are implemented as developed by the client, family, and team.

Responsibilities
  • Coordinate integrated services and supports to assist clients in achieving their recovery goals

  • Participate in staff meetings and trainings
Requirements

To be successful in this role, you will need to have a bachelor's degree in social work, psychology, or a related field, and at least 2000 hours of supervised experience in the delivery of mental health services to adults. You will also need to be skilled in identifying and assessing a wide range of recipient needs, and knowledgeable about local community resources.
