About the Role

Mental Health Resources is seeking a dedicated and compassionate Mental Health Case Manager to join our team. As a key member of our community-based services, you will play a vital role in supporting individuals recovering from mental illness.

Key Responsibilities
  • Coordinate integrated services and supports to assist clients in achieving their recovery goals.
  • Develop and implement Individual Community Support Plans that identify services and supports to enable clients to live independently in the community.
  • Provide education and collaboration to significant others and family members to support the client's recovery process.
  • Treat clients with dignity and respect, providing services that are respectful of diagnosis, age, gender, and culture.
  • Assure that services are implemented as developed by the client, family, and team, and continuously monitored and evaluated.
Requirements
  • Bachelor's degree in social work, psychology, or a human services-related field from an accredited college or university.
  • At least 2000 hours of supervised experience in delivering mental health services to adults.
  • Skilled in identifying and assessing a wide range of recipient needs.
  • Knowledgeable about local community resources and how to use them for the benefit of the recipient.
