Job Summary

Lantern Community Services Inc is seeking an experienced Case Manager to join our interdisciplinary team at Liberty Plaza. The successful candidate will have prior experience working with homeless and/or mental health individuals, and will be responsible for assessing client needs, linking clients to services, and planning a course of action towards stabilization and re-housing.

Key Responsibilities
  • Accomplish clients' care by assessing treatment needs, developing, monitoring, and evaluating treatment plans and progress.
  • Admit new clients by reviewing records and applications, conducting orientations, and determining clients' requirements by completing intake interviews.
  • Establish treatment programs by setting schedules and routines, coordinating services, arranging resources, and monitoring cases.
  • Communicate clients' progress by conducting weekly interdisciplinary meetings and evaluations, disseminating results and obstacles to the therapeutic team and family.
Requirements
  • Bachelor's Degree in Social Work or a related field.
  • Experience working with homeless, mentally ill, substance abuse, and/or low-income populations.
  • Strong relationship-building skills and ability to work evenings and/or weekends on a rotating schedule.
  • Preferred experience working in a homeless shelter and knowledge of mental health, substance use, or homeless services.
