Opportunity Youth Case Manager

Commonpoint Queens is seeking a highly motivated and experienced Opportunity Youth Case Manager to join our team. As a Case Manager, you will be responsible for providing case management services to opportunity youth, ages 17-24, who are out of school and out of work.
Job Summary:The Opportunity Youth Case Manager will work with a caseload of 30-35 participants, conducting intake assessments, assisting with eligibility documentation, and providing individual and group mental health support. The Case Manager will also monitor participant program performance and attendance, provide detailed case notes and data entry, and assist in the development and implementation of Individual Services Strategies for each participant.
Responsibilities:- Utilize a trauma-informed approach to working with opportunity youth
- Manage a caseload of 30-35 participants
- Conduct intake assessments and assist with eligibility documentation
- Provide individual and group mental health support
- Monitor participant program performance and attendance
- Provide detailed case notes and data entry
- Assist in the development and implementation of Individual Services Strategies for each participant
- Bachelor's degree in Human Services, Psychology, or related field
- 3+ years of case management experience with opportunity youth
- Trained in trauma-informed care, crisis intervention, and de-escalation
- Demonstrated ability in youth development and cultural competency
- Proven experience with participant navigator services
The Opportunity Youth Case Manager will work in a fast-paced environment, providing case management services to opportunity youth. The Case Manager will be required to travel to multiple service sites within the agency as needed.
