Job Title: Case Manager

We are seeking a skilled Case Manager to join our team at Lantern Community Services Inc. The successful candidate will be responsible for assessing client needs, linking clients to services, and planning a course of action towards stabilization and re-housing.

Key Responsibilities:
  • Accomplish clients' care by assessing treatment needs, developing, monitoring, and evaluating treatment plans and progress.
  • Admit new clients by reviewing records and applications, conducting orientations, and determining clients' requirements.
  • Establish treatment programs by setting schedules and routines, coordinating services, and arranging resources.
  • Monitor cases by verifying clients' attendance, observing and evaluating treatments and responses, and advocating for needed services and entitlements.
  • Communicate clients' progress by conducting weekly interdisciplinary meetings and evaluations, disseminating results and obstacles to the therapeutic team and family, and identifying treatment influences.
Requirements:
  • High school diploma, HSE or GED, required.
  • A minimum of 2 years of social services experience required.
  • Experience working with the homeless, mentally ill, substance abuse, and/or low-income populations.
  • Strong relationship-building skills and strong computer skills, including Word and Excel.
  • Ability to work evenings and/or weekends on a rotating schedule.
  • Preferred experience working in a homeless shelter and knowledge of mental health, substance use, or homeless services.
  • Bilingual (English/Spanish) preferred.
Work Environment:

This job operates in a shelter environment and works with a population who may suffer from mental illness, substance abuse, disability, HIV/AIDS, and/or other health issues.

Physical Demands:

While performing the duties of this job, the employee is regularly required to stand, walk, sit, stoop, climb stairs, and/or kneel. Occasionally the employee will have to lift and/or move up to 10 pounds.
