Case Manager I

Overview:

The Case Manager I role operates under the guidance of the Family Development Program Manager, delivering essential emergency and supportive services to individuals experiencing housing challenges in the community.

Key Responsibilities:

  • Evaluate clients' urgent situations and immediate requirements
  • Assist the Family Development team in their initiatives
  • Leverage resources from Community Action Services & Food Bank (CASFB)
  • Provide referrals to community resources
  • Engage and recruit eligible individuals for the program's case management services
  • Conduct follow-up consultations to ensure ongoing support
  • Organize family-oriented activities and document their outcomes
  • Accurately record services provided and their results
  • Collaborate with other CASFB staff to address community needs

Qualifications:

  • At least two years of post-secondary education in Human Services or Social Work
  • A minimum of two years of relevant work experience
  • Familiarity with resources available in the Family Development program and the wider community
  • Strong written and verbal communication abilities
  • Access to reliable transportation and willingness to work flexible hours
  • Spanish language proficiency is preferred
