Overview:
The Case Manager I role operates under the guidance of the Family Development Program Manager, delivering essential emergency and supportive services to individuals in need of housing assistance within the community.
Key Responsibilities:
- Evaluate clients' urgent situations and immediate requirements
- Assist the Family Development team in program initiatives
- Leverage resources from Community Action Services & Food Bank (CASFB)
- Provide referrals to community resources
- Engage and recruit qualified participants for case management services
- Conduct follow-up consultations to ensure client needs are met
- Organize family-oriented activities and track outcomes
- Maintain accurate documentation of services provided and outcomes achieved
- Collaborate with other CASFB staff to address community challenges
Qualifications:
- A minimum of two years of post-secondary education in Human Services or Social Work
- At least two years of relevant work experience in similar roles
- Familiarity with resources available in the Family Development program and local community
- Strong written and verbal communication abilities
- Access to reliable transportation and flexibility in scheduling
- Proficiency in Spanish is preferred
