Case Manager I

Overview:

The Case Manager I role operates under the guidance of the Family Development Program Manager, delivering essential emergency and supportive services to individuals in need of housing assistance within the community.

Key Responsibilities:

  • Evaluate clients' urgent situations and immediate requirements
  • Assist the Family Development team in program initiatives
  • Leverage resources from Community Action Services & Food Bank (CASFB)
  • Provide referrals to community resources
  • Engage and recruit qualified participants for case management services
  • Conduct follow-up consultations to ensure client needs are met
  • Organize family-oriented activities and track outcomes
  • Maintain accurate documentation of services provided and outcomes achieved
  • Collaborate with other CASFB staff to address community challenges

Qualifications:

  • A minimum of two years of post-secondary education in Human Services or Social Work
  • At least two years of relevant work experience in similar roles
  • Familiarity with resources available in the Family Development program and local community
  • Strong written and verbal communication abilities
  • Access to reliable transportation and flexibility in scheduling
  • Proficiency in Spanish is preferred
