Responsibilities:
- Perform admission, discharge, and routine home health visits to assess patient needs and develop personalized care plans.
- Coordinate and communicate with the interdisciplinary team to ensure seamless care delivery and effective patient outcomes.
- Document all phone calls and visits using our Electronic Medical Record (EMR) system.
Requirements:
- Current RN licensure and certification in case management.
- Minimum 2 years of experience in home health care or a related field.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
