Key Responsibilities
  • Coordinate patient care services, including admission and discharge, team conference, and interdisciplinary plan of care communication.
  • Manage issues related to patient and family education, payor relations, and coordination of care for all disciplines.
  • Perform ongoing utilization and quality reviews to ensure cost-effective treatment.
  • Act as a liaison between the payor and patient, ensuring that cost-effective treatment is provided by the team.
  • Support the fundamentals of safety, quality, experience, and stewardship by providing clinical expertise and leadership to the department.
  • Participate in the overall operations of the department, development of department practice, and provision of patient care.

Requirements
  • Current RN license in the state of practice.
  • Current Driver's License in the state of practice.
  • Current Hospice Certification or Oasis Certification designation must be achieved within 12 months of hire/transition into this position.
  • Basic Life Support (BLS).
  • Two years of clinical nursing experience.
  • One year of experience working in Home Health or Hospice setting.
  • Basic computer skills and knowledge of word processing software.
  • Excellent written and verbal communication skills.
  • Independent worker, who is self-motivated, has a positive attitude, and can be flexible in a rapidly changing healthcare environment.
