The Homecare RN Case Manager plays a vital role in the interdisciplinary team, overseeing the effective coordination of services and managing issues related to patient care. Key Responsibilities- Manage and direct resource utilization to achieve the highest quality outcomes during a patient care experience.
- Oversee the effective coordination of services, including admission and discharge, team conference, and interdisciplinary plan of care communication.
- Manage issues related to patient and family education, payor relations, and coordination of care for all disciplines.
- Perform ongoing utilization and quality reviews, ensuring cost-effective treatment is provided by the team.
- Act as a liaison between the payor and patient, ensuring seamless communication and coordination.
- Support the fundamentals of safety, quality, experience, and stewardship by providing clinical expertise and leadership to the department.
- Participate in the overall operations of the department, development of department practice, and provision of patient care.
- Current RN license in the state of practice.
- Current Driver's License in the state of practice.
- Current Hospice Certification or Oasis Certification designation must be achieved within 12 months of hire/transition into this position.
- Basic Life Support (BLS).
- RNs hired or promoted into this role need to have or obtain their BSN within three years of hire or promotion.
- Two years of clinical nursing experience.
- One year of experience working in Home Health or Hospice setting.
- Basic computer skills and knowledge of word processing software.
- Excellent written and verbal communication skills.
- Independent worker, who is self-motivated, has a positive attitude, and can be flexible in a rapidly changing healthcare environment.
- Record as a safe driver. Must meet Intermountain Driving requirements.
- Bachelor's degree in Nursing (BSN) from an accredited institution.
- Three years of experience working as a case/care manager in a home health or hospice setting.
- Experience working with third-party payers.
- Knowledge of home-based health services and resources.
