Job Summary:

The Alleghany Highlands Community Services is seeking a highly skilled and compassionate School Based Case Manager/Region 1-Emergency Crisis Counselor to join our team. As a key member of our crisis intervention team, you will provide critical support to individuals in the school setting who are at risk of developing or have mental health and/or substance abuse issues.

Key Responsibilities:

  • Assess and Develop Individual Service Plans: Conduct thorough assessments to identify client needs and develop individualized service plans in consultation with supervisory staff.
  • Provide Ongoing Case Management and Behavioral Interventions: Offer ongoing support and guidance to clients, based on assessed needs and goals, to promote positive behavioral change.
  • Implement Behavioral Management Programs: Develop and implement behavioral management programs as outlined in the service plan, ensuring a safe and supportive environment for clients.
  • Collaborate with Interdisciplinary Teams: Work closely with interdisciplinary teams, including educators, healthcare professionals, and social workers, to ensure comprehensive support for clients.

Requirements:

  • Education: Bachelor's Degree in a relevant Human Services field (required)
  • Certification: QMHP-C or QMHP-Trainee (required)
  • License: Valid Driver's License (required)
