Position Overview: The School-Based Case Manager/Region 1 - Emergency Youth Crisis Counselor is a versatile role aimed at supporting students in educational environments who may be at risk of or currently experiencing mental health and/or substance use challenges. This position encompasses a range of services including evaluations, supportive counseling, advocacy, and connections to necessary resources as outlined in the Individual Service Plan (ISP).
This role is committed to delivering high-quality support while adhering to safety protocols and continuously enhancing knowledge to assist individuals with their diverse needs. Additional responsibilities include conducting pre-admission assessments, evaluating crisis situations, employing de-escalation techniques, and providing supportive counseling and advocacy as per the ISP requirements.
Qualifications:
- Bachelor's Degree in a relevant Human Services discipline (mandatory)
- QMHP-C or QMHP-Trainee certification (mandatory)
- Valid Driver's License (mandatory)
Key Responsibilities:
- Deliver ongoing case management and behavioral interventions tailored to the assessed needs and objectives of clients.
- Support, monitor, and engage with clients in the day treatment program, implementing behavioral management strategies as specified in the service plan with professionalism and integrity.
- Collaborate with supervisory personnel to develop Individual Service Plans based on thorough assessments within a team-oriented service delivery framework.
