Position Overview: The School-Based Case Manager for Region 1 serves as an Emergency Crisis Counselor, operating in a hybrid capacity to support students within the educational environment who may be at risk of or currently experiencing mental health and/or substance use challenges. The role encompasses a variety of services including comprehensive assessments, supportive counseling, advocacy, and referrals to essential services as outlined in the Individual Service Plan (ISP).

This position is committed to delivering high-quality support while adhering to safety protocols and continuously enhancing knowledge to meet the diverse needs of individuals. Additional responsibilities include conducting pre-admission evaluations, assessing crisis scenarios, implementing de-escalation techniques, and facilitating access to necessary services as specified in the ISP.

Qualifications:

  • A Bachelor's Degree in a relevant Human Services discipline is required.
  • Certification as a QMHP-C or QMHP-Trainee is mandatory.
  • A valid Driver's License is essential.

Key Responsibilities:

  • Deliver ongoing case management and behavioral interventions tailored to the assessed needs and objectives of clients.
  • Support, monitor, and engage with clients in the day treatment program, executing behavioral management strategies as detailed in the service plan with professionalism and integrity.
  • Collaborate with supervisory personnel to develop Individual Service Plans based on comprehensive assessments within a team-oriented service delivery framework.
