Liberty Hospice Care Coordinator

As a Liberty Hospice Care Coordinator, you will play a vital role in delivering comprehensive care to our patients and their families. This position requires a compassionate and skilled professional who can provide direct patient care services and document patient care data.
Responsibilities
- Conduct clinical assessments to develop individualized care plans and set treatment goals.
- IDentify and assess social, emotional, financial, and environmental factors affecting patient care.
- Communicate effectively with healthcare teams and coordinate services to optimize patient care.
- Provide bereavement support and grief care to patients and caregivers.
Requirements
• Advanced degree in Social Work (MSW) or related field
• Minimum one year experience in home health/hospice care environment
• Excellent communication and interpersonal skills
Estimated Salary: $72,000 - $90,000 per year
