We are currently seeking an experienced Hospice Medical Social Worker to join our team. This role will involve providing direct patient care services, documenting patient data, and developing individualized care plans.

Key Responsibilities:

  • Provide clinical assessments for patient care planning
  • Apply a working knowledge of psycho-social principles to identify factors affecting medical treatment
  • Communicate effectively with agency personnel and coordinate care
  • Provide bereavement support and grief care to patients and caregivers

Requirements:

  • Masters of Social Work degree from a Council on Social Work Education accredited school
  • Minimum one year practical experience in home health/hospice care environment
  • Strong interpersonal skills and ability to communicate within multidisciplinary setting
