Hospice Medical Social Worker

**Job Title:** Hospice Medical Social Worker - Patient Care Specialist
We are seeking an experienced and compassionate hospice medical social worker to join our team at Liberty HomeCare. In this role, you will provide direct patient care services and document patient care data.
**About the Role:*
- Provide clinical assessment for individualized care planning and set treatment goals.
- Apply a working knowledge of psycho-social principles and family dynamic development.
- Evaluate bereavement needs of families through initial assessments and interdisciplinary team coordination.
**Requirements:
- Masters of Social Work degree from a Council on Social Work Education accredited school with one year of experience in home health/hospice care.
- Strong interpersonal skills and ability to communicate effectively within a multidisciplinary setting.
The estimated salary for this position is $65,000-$85,000 per year, depending on location and experience.
