Core Objective of This Role

The core objective of this role is to deliver direct nursing care to residents while overseeing nursing operations during the designated shift.

Key Responsibilities

Administrative Duties
  • Ensure the facility maintains adequate staffing levels for nursing personnel; complete necessary postings of nursing staff information as required.
  • Conduct assessments for residents experiencing significant changes in condition using the Resident Assessment Instrument (RAI) process.
  • Maintain cleanliness and safety in resident rooms, common areas, treatment zones, and nursing stations.
  • Assist in the creation of a dementia care program that utilizes an interdisciplinary approach focused on the holistic needs of residents with dementia.
  • Verify the accuracy of resident census and report new admissions, discharges, and other relevant changes according to facility protocols.
  • Facilitate safe and organized resident transfers and discharges in compliance with facility policies.
  • Coordinate pre-certifications and recertifications as per facility guidelines for Medicare Advantage and commercial insurance providers.
  • Interpret departmental policies and procedures to staff, residents, visitors, and regulatory agencies as necessary.
  • Engage in the development and revision of written policies and procedures governing nursing services.
  • Participate in facility inspections conducted by authorized government entities as requested.
  • Audit nursing documentation for accuracy and relevance in clinical records.
  • Contribute to the facility's review process for coding accuracy.
  • Perform administrative tasks such as completing medical forms, reports, and evaluations as needed.
  • Review departmental manuals and job descriptions periodically, recommending updates to the Director of Nursing.
  • Coordinate the completion of resident billing documentation in line with billing policies.
Committee Engagement
  • Prepare reports as needed.
  • Attend various facility committee meetings (e.g., infection control, policy advisory, quality assurance) as required.
  • Assist in developing agendas for committee meetings as necessary.
  • Participate in regular reviews of discharge plans.
Nursing Care Responsibilities
  • Deliver licensed nursing care to residents in accordance with established care plans.
  • Communicate with primary care physicians and members of the care planning team regarding resident status; follow up on critical lab results and physician notifications.
  • Arrange for interpreter services for residents with limited English proficiency as per facility guidelines.
  • Obtain orders for essential resident care upon admission, consistent with their mental and physical status.
  • Ensure all residents are treated with dignity and respect, allowing them to exercise their choices and self-determination.
  • Provide culturally competent, trauma-informed care in line with professional standards.
  • Evaluate residents' responses to care.
  • Coordinate nursing care to align with each resident's person-centered care plan, minimizing disruptions to their activities.
  • Ensure residents are free from unnecessary restraints that are not medically required.
  • Facilitate coordination of nursing services with other resident services to ensure continuity of care.
  • Identify and address the educational needs of residents and families, providing tailored education.
  • Educate residents and families on treatment risks and benefits when consent is necessary.
  • Monitor residents for signs of distress or decreased social interaction.
  • Implement individualized dementia care strategies that include meaningful activities reflecting residents' preferences.
Additional Responsibilities
  • Conduct restorative and rehabilitative programs, including teaching activities of daily living and self-help education.
  • Administer pain management in accordance with established protocols.
  • Complete incident reports as necessary.
  • Assist the interdisciplinary team in establishing nutrition and hydration parameters for residents.
  • Keep the Director of Nursing informed of resident statuses and related matters through written and oral reports.
  • Coordinate social and activity programs with nursing schedules.
  • Review and update residents' discharge plans as needed.
  • Ensure that discharge needs are identified and addressed through comprehensive planning.
  • Monitor Certified Nursing Assistants (CNAs) to ensure proper treatment administration and evaluate residents' statuses.
  • Report issues to the Director of Nursing and assist in corrective action development.
  • Arrange for diagnostic and therapeutic services as ordered by physicians.
  • Provide information to the Quality Assurance and Performance Improvement Committee as requested.
  • Coordinate resident transfers or discharges, ensuring safe transportation and comprehensive discharge summaries.
  • Ensure discharged residents are escorted to the designated pick-up area.
Documentation and Charting Duties
  • Complete and file necessary recordkeeping forms upon resident admission, transfer, or discharge.
  • Document incidents involving residents following established procedures.
  • Perform routine charting duties in accordance with documentation policies.
Medication Administration Responsibilities
  • Review controlled substance records for accuracy and report discrepancies.
  • Monitor the medication administration process and provide feedback to prevent errors.
  • Ensure medication carts and rooms are clean and well-stocked.
  • Assess residents for appropriate self-administration of medications.
  • Verify medication deliveries from the pharmacy for accuracy.
  • Collaborate with the interdisciplinary team to ensure safe medication use.
  • Administer medications according to physician orders and facility policies.
  • Support infection prevention initiatives, including antibiotic stewardship.
  • Review unnecessary medications and coordinate gradual dose reductions as appropriate.
  • Reconcile medications during the discharge process.
  • Order necessary medications and supplies in accordance with facility policies.
Personnel Management Responsibilities
  • Meet with nursing staff and support personnel to plan services and identify areas for improvement.
  • Assist the Director of Nursing in managing daily nursing operations per regulations and guidelines.
  • Direct the daily functions of CNAs in compliance with facility standards.
Additional Duties
  • Ensure services are delivered by qualified individuals with the necessary skills and certifications.
  • Assess residents for assistive device needs and monitor their use.
  • Supervise nurse aide trainees under licensed nurse guidance.
  • Foster positive relationships with nursing students and educators.
  • Prepare and deliver nursing reports during shift changes.
  • Assist in the grievance process, coordinating responses to resident and family concerns.
  • Review complaints made by assigned personnel and report to the Charge Nurse as necessary.
Staff Development Responsibilities
  • Collaborate with the Staff Development Coordinator for team training and skill development.
  • Assist in training department personnel.
  • Support the Director of Nursing in planning clinical supervision for trainees and new hires.
  • Participate in continuing education programs to maintain nursing competencies.
Safety and Sanitation Responsibilities
  • Ensure compliance with departmental policies and procedures among nursing staff.
  • Monitor the use of safety equipment during resident handling.
  • Maintain a safe environment for residents, minimizing hazards.
  • Ensure adherence to infection prevention protocols.
Additional Responsibilities
  • Participate in emergency preparedness drills and ensure staff compliance.
  • Monitor adherence to safety protocols during equipment use.
  • Contribute to the development of procedures for reporting hazardous conditions.
Equipment and Supply Management
  • Maintain lists of equipment repair needs for resident care.
  • Recommend equipment and supply needs to the Charge Nurse.
  • Ensure safety data sheets are available for hazardous chemicals.
  • Participate in developing safe operation procedures for nursing equipment.
Miscellaneous Responsibilities

Complete tasks assigned by the Quality Assurance and Performance Improvement Committee within specified timeframes.

Education Requirements

Must possess a nursing degree from an accredited institution or be a graduate of an approved LPN/LVN program.

Experience Requirements
  • Minimum of one year of supervisory experience in a healthcare setting.
  • Training in rehabilitative and restorative nursing practices is required.
