Position Overview

As a Case Management Assistant at Richmond University Medical Center, you will play a crucial role in ensuring the smooth and efficient transition of patients from acute care settings to various alternative care options. This includes skilled nursing facilities, long-term acute care, inpatient rehabilitation, home infusion therapy, hospice, and home care programs.

Key Responsibilities
  • Facilitate safe and timely patient transitions in accordance with the inpatient discharge care plan.
  • Engage in effective communication with patients and collaborate with all members of the healthcare team.
  • Coordinate services with post-acute providers and medical insurance payors.
  • Manage transportation arrangements and obtain necessary authorizations for the Care Coordination department.
  • Deliver regulatory notices as assigned by the Care Coordination Team.
Qualifications

The ideal candidate will possess:

  • A minimum of 2-3 years of experience in healthcare roles such as Nursing Assistant, Medical Assistant, Health Unit Coordinator, or Placement Specialist.
  • Strong communication and customer service skills.
  • Effective human relation skills and time management abilities.
  • Flexibility to respond to multiple demands and work weekends on a rotational basis.
Compensation

Salary Range: $26.074/hr - $27.430/hr
