Department: Transitional Housing

Reports to: Social Service Supervisor

FLSA Status: Non-Exempt

Compensation: $24.49 per hour

Role Overview:

The Case Manager plays a vital role in delivering comprehensive case management services to individuals in transitional housing. This position involves managing a designated caseload and focusing on the integration of housing and employment objectives for each client while ensuring adherence to the organization's contractual commitments.

Key Responsibilities:

  • Perform initial evaluations and complete necessary documentation upon client intake.
  • Facilitate referrals for urgent services.
  • Support clients with orientation and essential housekeeping tasks.
  • Provide crisis intervention, counseling, and supportive services.
  • Create and monitor Independent Living Plans (ILP) to track client progress.
  • Assist clients in securing income and benefits.
  • Connect clients with medical, mental health, and community resources as required.
  • Participate in meetings and training sessions as mandated.
  • Prepare and submit required reports.

Essential Functions:

  • Ability to navigate stairs within the facility.
  • Accompany clients to appointments and viewings.

Qualifications:

  • Bachelor's Degree in social work or a related discipline is preferred.
  • Experience with evidence-based practices and standardized assessment tools.
  • Strong communication and organizational skills.
