About the Role

We are seeking a highly skilled Care Services Coordinator to join our team at Arkos Health. As a Care Services Coordinator, you will play a critical role in facilitating care for patients by coordinating and scheduling with our field providers.

Key Responsibilities
  • Provide exceptional phone support to our transitional/post-acute teams, managing patient needs through inbound and outbound call management.
  • Explain our services to patients and obtain verbal consent for transitional/post-acute team and clinical services.
  • Coordinate and manage schedules for our transitional/post-acute and clinical services teams, ensuring timely delivery of services.
  • Act as a liaison with referral sources, transitional/post-acute team, clinical services, and health plans to coordinate orders, supplies, appointments, and medical records.
  • Upload orders, field documents, and requested records to patient charts for the clinical services team.
Requirements
  • High school diploma or GED required.
  • Some college or medical certification preferred.
  • Previous customer service experience preferred.
  • MA/CNA preferred but not required.
  • Healthcare experience preferred.
  • Knowledge of medications, medical terminology, and patient care preferred.
  • Bilingual is a huge plus.
