Position Overview

The Client Care Coordinator collaborates closely with the care management team to evaluate clients, develop and execute personalized care plans, conduct regular home assessments, and supervise client care. The team is responsible for credentialing caregivers and their environments while providing continuous support and training.

Key Responsibilities

  • Conduct initial home evaluations, gather necessary caregiver credentialing documents, and ensure completion of required home modifications.
  • Collect social histories and perform psychosocial assessments.
  • Assist in gathering information and making referrals to other healthcare and community resources.
  • Coordinate client placement after program acceptance, ensuring adherence to all necessary documentation and procedures.
  • Identify significant changes in client status and collaborate with the care management team for required assessments.
  • Provide ongoing coaching to enhance the effective use of established best practices and protocols.
  • Work together to develop and implement the client plan of care (POC), monitoring and updating it according to company and regulatory standards.
  • Oversee client care through consistent communication and home visits, including the review and supervision of personal care services.
  • Document care management progress notes corresponding to each home visit or significant encounter.
  • Deliver client-specific training to caregivers regularly and maintain records of training and caregiver competency.
  • Review case documentation daily for accuracy, completeness, compliance, and timeliness, following up as necessary.
  • Engage in regular communication with the clinical team, agency, support services, primary care physicians, and other medical providers to ensure timely and appropriate service delivery.
  • Participate in discharge and transition planning and execution.
  • Conduct annual reviews of the suitability for the qualified setting.
  • Ensure ongoing readiness for surveys and compliance with all regulatory and accreditation standards.
  • Perform additional duties as assigned.

Qualifications

  • Bachelor's degree and a minimum of two years of clinical experience working with seniors or adults with disabilities, or an equivalent combination of education and experience, including internships in related fields.
  • Ability to travel within the assigned geographic area; a valid driver's license and automobile insurance are required.
  • Experience in case management, care planning, and assessment for cases involving eldercare, individuals with disabilities, and other complex medical needs in a healthcare environment.
  • Proficiency in common software and web-based applications (e.g., Case Management Systems, Office).
  • Additional state-specific qualifications may be necessary.
