Job Title: Clinical Evaluator

We are seeking a skilled and compassionate Clinical Evaluator to join our team at The Pines at Rutland Center for Nursing & Rehabilitation.

Job Summary

The Clinical Evaluator will be responsible for conducting thorough assessments of residents' health conditions, functional abilities, and care requirements. This role will drive the development of personalized care plans, ensuring the highest standard of care and well-being of our residents.

Key Responsibilities
  • Conduct comprehensive assessments of physical, mental, and emotional health to determine appropriate care setting
  • Evaluate functional abilities and care needs through detailed observations and assessments
  • Collaborate with interdisciplinary teams to develop individualized care plans
  • Develop and maintain positive relationships with area hospitals and community agencies
  • Establish and maintain relationships with social workers, case managers, and other contacts in the healthcare and payor community
  • Educate residents and families about facility services, procedures, and care expectations
  • Monitor and reassess residents' progress and update care plans accordingly
  • Advocate for residents' well-being and coordinate with healthcare providers to address needs
  • Document assessments and care plans accurately and timely

Requirements
  • Valid state RN license preferred
  • 3 years of clinical or case management experience in long-term care
  • Experience in discharge planning and marketing activity
  • Compassionate and empathetic approach to patient care
  • Strong clinical skills and critical thinking abilities
  • Excellent communication and interpersonal skills
  • Ability to work effectively in a multidisciplinary team environment
