Role Summary

The Clinical Nurse Specialist in our clinic will engage in Triage Nursing and provide support across all clinic locations. This position is integral to delivering compassionate care to diverse populations, including those who are underserved.

Key Responsibilities

As a Clinical Nurse Specialist, you will:

  • Deliver direct nursing care in alignment with established protocols and policies.
  • Develop and oversee patient care plans, ensuring effective monitoring and communication of patient conditions.
  • Act as the primary coordinator for interdisciplinary collaboration to ensure seamless patient care.
  • Execute physician and nursing directives accurately.
  • Assess and facilitate patient discharge planning in collaboration with the healthcare team.
Qualifications

Licensure / Certification / Registration:

  • Current BLS Provider certification required within 30 days of hire or transfer.
  • Registered Nurse credential from the Indiana Board of Nursing required prior to employment.

Education:

  • Diploma from an accredited nursing school or equivalent professional licensure at the time of hire.
Preferred Qualifications

We strongly prefer candidates with:

  • A minimum of 2 years of recent experience in Family Medicine or Internal Medicine.
  • Proficiency in Electronic Medical Records (EMR) systems.
  • Experience in telephonic triage and strong telephonic customer service skills.
  • Exceptional assessment, critical thinking, and judgment capabilities.
