At MountainView Hospital, we are committed to delivering high-quality, patient-centered care. As a Case Manager, you will play a critical role in ensuring that our patients receive the best possible care and support throughout their rehabilitation journey.
Key Responsibilities- Develop and implement individualized treatment plans that support patients' strengths, abilities, needs, and preferences.
- Collaborate with the rehabilitation team to ensure seamless care coordination and effective communication.
- Conduct discharge planning evaluations and psychosocial assessments to identify patients' social, financial, and discharge needs and preferences.
- Communicate with patients, families, and caregivers to ensure their involvement and support throughout the rehabilitation process.
- Associate Degree in Nursing or Nursing Diploma required; Bachelor's Degree in Nursing preferred.
- At least 2+ years of experience in case management or 3+ years of experience in clinical nursing.
