MDS Coordinator Role and Responsibilities

  • As a Long Term Care Clinical Coordinator, you will develop and implement treatment plans to improve patient outcomes in our skilled nursing facility.
  • You will assess and evaluate the quality of care provided to residents in your assigned nursing home.
  • You will collaborate with nursing staff, patient caregivers, and family members to discuss resident conditions and treatment plans.
  • You will review and approve resident applications for admission to your nursing home.

Requirements and Qualifications

  • A Bachelor's degree in Nursing (BSN).
  • A current, valid license to practice as a registered nurse in Illinois.
  • At least 2 years of experience working in a long-term care facility.
  • Previous experience in an MDS Coordinator role is preferred.
  • Excellent communication and organizational skills.
