Nursing Care Coordinator

We are seeking a skilled RN Specialist to join our team at Bethesda Health, where you will play a vital role in delivering exceptional patient care.
About the JobIn this role, you will assess residents' health and educational needs, develop comprehensive care plans, and closely monitor their conditions. You will work collaboratively with interdisciplinary teams to communicate significant changes and deliver effective nursing interventions while ensuring thorough documentation.
Your Key Responsibilities- Accurately assess residents' health status, educational needs, and environmental requirements upon admission and throughout their stay.
- Communicate significant changes in condition to the appropriate interdisciplinary team members.
- Develop dynamic care plans based on identified resident needs through an interdisciplinary approach.
- Review and revise care plans as resident needs, problems, and goals evolve.
- Document all assessments, care plans, and interventions accurately and timely.
You will enjoy a competitive salary of $65,000 - $85,000 per year, depending on experience, plus excellent benefits, including:

- An accredited school of nursing degree resulting in a registered nurse license.
- A current RN license in the state where you will be working.
- CPR/AED certification or willingness to obtain within six months of employment.
- Long-term care experience is preferred but not required.
- Critical thinking skills and excellent communication, time management, and organizational abilities.
- The ability to stand and walk for extended periods.
- The capacity to lift heavy objects using proper techniques and assistance when necessary.
- The ability to transfer and move patients who require assistance due to their weight.
- Manual dexterity and coordination for interacting with patients and equipment.
