We are seeking a compassionate Registered Nurse to join our team as a RN Care Coordinator. In this role, you will play a vital part in providing patient-centered care in Belleville and surrounding areas. As a care coordinator, you will collaborate with our care team to develop and implement patient care plans that cater to each individual's needs.

The ideal candidate will have a RN license in the state of employment, one year of experience, and the clinical competence to deliver quality patient care. You will also be required to maintain a valid driver's license and spend approximately 20% of your day driving to/from patient locations.

This position offers a competitive salary range of $80,000 - $110,000 per year, based on productivity. Additionally, you will be eligible for a $10,000 first year success bonus upon meeting key achievements during your first year.

We are looking for nurses who:

  • Are licensed as an RN in the state of employment.
  • Have at least one year of experience as an RN.
  • Have the clinical competence to deliver quality patient care.
  • Hold a valid driver's license and can spend approximately 20% of their day driving to/from patient locations.
  • Can consistently meet critical deadlines for charting.
  • Can self-manage their time and schedule.
  • Have demonstrated experience with tablets, mobile phones, and EMR software.
