Position Summary: We are seeking a full-time Case Manager at our Bedrock Recovery Center. This role is essential in providing comprehensive services to individuals and families facing complex physical or mental health challenges. The Case Manager will identify the most effective treatment paths, helping clients achieve a better quality of life by understanding their needs and aspirations while collaborating with relevant contacts. This position reports directly to the Program Coordinator.
Key Responsibilities:
- Facilitate psycho-educational groups and individual case management sessions as required.
- Conduct thorough intake assessments.
- Lead treatment team meetings and family discussions.
- Manage a designated caseload, prioritizing clients' urgent needs.
- Perform detailed mental or physical health evaluations to create actionable plans, set objectives, and identify necessary resources.
- Provide counseling in both group and individual formats.
- Collaborate with external agencies to enhance support services and resources.
- Maintain detailed patient records documenting clients' progress throughout their treatment.
- Develop comprehensive discharge plans tailored to client needs.
- Execute additional duties as assigned.
Qualifications Required:
- Familiarity with Federal, State, and Municipal regulations governing addiction treatment, including confidentiality, patient rights, and professional ethics.
- Understanding of Drug-Free Workplace Policies and Workplace Violence protocols.
- Knowledge of addiction issues and the principles of the 12-Step Recovery Program.
- Experience with patient care documentation and histories.
- Proficiency in crisis intervention techniques and emergency procedures related to substance overdose, including First Aid and CPR.
- Awareness of the effects of alcohol and addictive substances on individuals and their families.
- Strong communication, observational, and writing skills.
Preferred Qualifications:
- LADC-2 Certification.
- 3-5 years of experience in case management within a behavioral health context.
Skills and Competencies:
- Problem-solving, stress management, ethical decision-making, and team collaboration.
- Ability to work with a multidisciplinary team to support various aspects of the recovery program related to physical health.
- Commitment to maintaining quality, safety, and infection control standards.
- Skill in observing, assessing, and documenting symptoms, reactions, and progress.
- Capability to provide basic therapeutic care or intervene appropriately for the population served.
- Ability to address the daily needs of patients with mental, emotional, and/or co-occurring diagnoses.
Educational Background:
- Bachelor's Degree preferred.
Physical Requirements: The role may require exerting up to 50 pounds of force occasionally and/or up to 20 pounds of force frequently.
