Job Description

At RADIAS Health, we are seeking a highly skilled Case Manager to join our Waivered Services Case Management team. As a Case Manager, you will play a critical role in assisting clients on a waiver to access needed services, including medical, social, educational, and other services. Your primary responsibility will be to support clients who have been deemed at risk of placement in a Nursing Facility, Neurobehavioral Hospital, or ICF/DD, and to set up and coordinate the services and medical equipment necessary for them to live in the least restrictive setting possible.

Key Responsibilities:

  • Assist clients on the CADI, BI, EW, and DD waivers in Hennepin County to access needed services.
  • Develop and implement a plan of care (CSSP) based on the MNChoices assessment results and within the county-approved allocation for the client.
  • Inform clients and/or their legal guardians of service options and assist clients in identifying potential providers and accessing services.
  • Plan and arrange for services and inform the county waiver contact of the proposed services to be authorized.
  • Provide services at the client's residence or other community locations, as specified in the CSSP.
  • Provide crisis intervention services, as needed, including referrals to meet emergency needs for housing, food, clothing, or financial assistance.
  • Respond to all Vulnerable Adult reports in a timely manner and document all responses for review by Hennepin County.
  • Determine client needs for residential treatment and select appropriate placement homes or facilities.
  • Monitor client progress towards established service goals and determine termination/continuation of service.
  • Consult with departmental staff, other county departments, and community service professionals to develop and provide appropriate and timely services.
  • Maintain accurate case records and complete forms for documentation of eligibility, treatment cost reimbursements, service referrals, evaluation of client outcomes, and general case management purposes.
  • Track and provide data used to monitor trends and developments relating to client and program needs, and to monitor agency outcomes.
  • Track billable time worked, by client, for reimbursement.

Requirements:

  • Bachelor's degree in behavioral sciences or a related field OR
  • Bachelor's degree in any major and one year of experience as a social worker, case manager, or care coordinator.
  • Must have a car, auto insurance, and a driver's license.
