Job Summary:

RADIAS Health is seeking a skilled Case Manager to join our Targeted Case Management team. As a Case Manager, you will provide person-centered services to promote recovery and community integration of adults with serious persistent mental illnesses and co-occurring substance use disorders within Ramsey County.

Key Responsibilities:

  • Develop and implement treatment plans based on diagnostic and functional assessments approved by clinical supervisors.
  • Provide or ensure access to needed medical, vocational, social, educational, financial, legal, and housing services.
  • Develop Individual Community Support Plans for each client every 6 months.
  • Schedule face-to-face visits as dictated by the ICSP.
  • Make appropriate referrals and assist clients in gaining access to needed services.
  • Participate in the commitment process as mandated.
  • File all legally required reports.
  • Assume responsibility for providing services necessary to maintain and enhance each client's ability to live independently.

Requirements:

  • Bachelor's degree in behavioral sciences or related field (or 30 semester credit hours in behavioral health sciences) and 2,000 hours supervised work experience with individuals diagnosed with serious mental illness and/or substance use disorders.
  • OR Master's or other graduate degree in behavioral health sciences or related field.
  • OR 4,000 hours supervised work experience with individuals diagnosed with serious mental illness and/or substance use disorders.
