About the Role

Guild is seeking a skilled Case Manager to join our team in providing mobile case management services to individuals experiencing homelessness. As a Case Manager, you will work closely with clients to assess their needs, develop treatment goals, and provide support to achieve housing stability and overall well-being.

Key Responsibilities
  • Establish therapeutic relationships with clients and conduct assessments to identify strengths and areas for growth
  • Develop and implement individualized treatment plans to address clients' needs and goals
  • Provide housing support and connect clients with community and healthcare resources to achieve housing retention
  • Utilize evidence-based practices, including Housing First, Motivational Interviewing, and Harm Reduction, to support clients' recovery
  • Facilitate services with clients' community treatment teams and provide crisis intervention as needed
  • Maintain timely and accurate documentation of client services and progress
Requirements
  • Bachelor's degree in a behavioral science or related field from an accredited college or university
  • At least 2,000 hours (1 year) of experience in delivering services to individuals with mental illness
  • OR qualified as a mental health practitioner in the state of Minnesota with at least 30 semester/45 quarter credits and 2,000 hours of supervised experience in providing mental health services
Preferred Qualifications
  • Experience providing Targeted Case Management services
  • Experience providing homeless outreach services and/or serving people experiencing homelessness
  • Knowledge of income, benefits, and supports available to individuals served through federal, state, and county programs
