Job Description

Care Comfort Hospice is seeking a highly skilled and compassionate Registered Nurse Case Manager to join our team. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality, patient-centered care to individuals and their families.

Responsibilities
  • Coordinate and provide comprehensive hospice care to patients and families, ensuring a seamless transition from acute care to palliative care.
  • Assess and coordinate pain and symptom management programs, working collaboratively with patients, families, physicians, and the interdisciplinary team.
  • Facilitate freedom of choice, relief of suffering, and death with dignity for patients facing end-of-life issues.
  • Provide education and support to families based on the hospice philosophy of care, addressing physical, psychological, spiritual, and financial comfort.
  • Participate in interdisciplinary team meetings and update the Plan of Care (POC) for assigned patients based on identified problems and needs.
  • Ensure all services provided are in accordance with the patient's POC, promoting a patient-centered approach to care.
  • Communicate effectively with patients, families, and healthcare professionals to ensure a cohesive care plan.
  • Collaborate with the Director of Nursing and Executive Director to ensure seamless care coordination and patient outcomes.
Requirements
  • Current licensure as a Registered Nurse in the state of Pennsylvania.
  • One to two years of experience in nursing, preferably in an acute care setting or home health, hospice, or a comparable environment.
  • Ability to work independently, make judgments based on assessments and data available, and act accordingly.
  • Flexibility, innovation, and excellent interpersonal skills.
  • Emotional stability and the ability to cope with mental and emotional stress.
  • Valid driver's license, auto liability insurance, and reliable transportation.
