As a skilled Registered Nurse with JM Chandler Dream Builders, you will play a vital role in delivering high-quality patient care in the comfort of their own homes. This Evening Home Health Registered Nurse position requires a compassionate and detail-oriented individual who can manage and deliver patient care plans with precision and care.
Key Responsibilities- Lead and Collaborate with our interdisciplinary team to develop and implement patient care plans that meet the unique needs of each individual.
- Provide Management and Delivery of patient care services in a home-based setting, ensuring that patients receive the highest level of care and support.
- Meet Critical Deadlines for charting and documentation, maintaining accurate and up-to-date records of patient care.
- Self-Manage Time and Schedule to ensure efficient use of time and resources, prioritizing tasks and managing workload effectively.
- Associate Degree or Higher in Nursing or related field.
- 1+ Years of Experience in home health care or a related field, with a strong understanding of Electronic Medical Records (EMR) and other relevant systems.
- Valid Registered Nurse (RN) License in the state of Pennsylvania (PA).
- Valid Regular Driver License (DL) and a reliable vehicle for travel.
- Current Licenses/Certifications in American Heart Association Basic Life Support (AHA BLS) and American Heart Association Advanced Cardiac Life Support (AHA ACLS).
- Excellent Communication and Interpersonal Skills, with the ability to work effectively with patients, families, and healthcare teams.
- RN License Number for verification purposes.
