We seek an experienced Registered Nurse to assume primary responsibility for patient and family caseload, including assessment, eligibility determination, admission processing, planning, implementation, and evaluation phases of the nursing process.
Key Responsibilities- Obtain and assess necessary information from patient physical, psychological, social, and spiritual factors to initiate intervention by the interdisciplinary team
- Communicate with attending physicians, the interdisciplinary team, and other agencies to coordinate optimal care
- Provide regular visits to reassess patient needs and update the plan of care
- Perform clinical pronouncement of death and offer support to families
- Maintain accurate patient records, charts, progress notes, reports, and prepare care plans timely
- Supervise and document delivery of care by Home Health Aides as required by Federal, State, and Company guidelines
- Graduate from an accredited registered nursing program
- Licensed as a registered nurse in the state of practice
- Current CPR and First Aid Certification
- Knowledgeable of quality assessment and assurance procedures
- Valid driver's license and proof of insurance
We offer a competitive salary range of $85,000 - $105,000 per year, depending on experience, plus comprehensive benefits and a 401(k) retirement plan with a generous company match. Estimated annual salary based on location and experience is around $90,000.
