Job Summary

We are seeking a compassionate and skilled RN Care Manager to join our team at Ohio's Hospice, Inc. This is a full-time exempt position with a competitive salary and excellent benefits package.

Estimated Salary: $65,000 - $85,000 per year

As an RN Care Manager, you will be responsible for planning and delivering high-quality care to patients utilizing the nursing process of assessment, planning, interventions, implementation, and evaluation. You will work closely with patients, families, and other healthcare professionals to ensure seamless coordination of services.

Key Responsibilities
  • Conduct thorough assessments of patients' physical, spiritual, and psychosocial needs.
  • Develop and implement individualized plans of care, setting measurable patient-centered goals in collaboration with patients and families.
  • Review and update plans of care regularly to reflect changes in patient needs and provide high-quality care.
  • Collaborate with physicians and other healthcare professionals to determine eligibility for services and facilitate smooth transitions.
  • Document all interactions and updates thoroughly in the electronic health record to ensure accurate information and safe delivery of services.
Qualifications
  • Graduate of an accredited nursing school; Associate of Science in Nursing (ASN) or Bachelor of Science in Nursing (BSN) preferred.
  • Active Registered Nurse license in the State of Ohio without any board actions.
  • Two years of acute care nursing experience preferred.
  • Certification in Hospice & Palliative Medicine (CHPN) preferred; required within two years of employment.
  • CPR Certified.
