About the Role

We are seeking a skilled Hospice Care Specialist to join our team at Affinity Care of Ohio.

Job Summary

The Hospice Care Specialist will work under the supervision of a registered nurse, performing various tasks as assigned in accordance with the patient's plan of care. The successful candidate will be responsible for reporting and documenting observations and care performed.

Key Responsibilities
  1. Personal Care: Provide personal care needs after receiving patient assignments and attend to requests promptly. Report any inability to perform tasks to the Case Manager immediately.
  2. Communication: Employ positive communication techniques, adhering to ethical and respectful care principles, including confidentiality.
  3. Safety Needs: Meet patient safety needs, using equipment safely and properly.
  4. Personal Hygiene: Give personal care, including baths, back rubs, oral hygiene, shampoos, and changing bed linen as often as assigned.
  5. Dressing and Feeding Assistance: Assist patients with dressing and feeding as assigned.
  6. Patient Observation: Provide proper care and observation of patient skin to prevent breakdown over bony prominence.
  7. Reporting Symptoms: Report any complaint of pain or other symptoms or changes in the patient's condition to the RN Case Manager.
  8. Ambulation and Exercise: Assist with ambulation and exercise as instructed by the hospice nurse or therapist.
  9. Normal Range of Motion: Provide normal range of motion and transfers/positioning as directed.
  10. Medication Assistance: Assist with self-administered medications as allowed by state regulations.
  11. Light Laundry and Cleaning Tasks: Offer assistance with light laundry needs, emptying waste baskets, and other light homemaker tasks.
  12. Procedures and Respite: Perform simple procedures as ordered, complete skills competency, and provide respite for patients/families/caregivers when on-site.
Requirements
  • Completion of a state-approved certified nursing assistant course or equivalent training.
  • Current Healthcare Provider CPR certification (may not be internet-based unless hands-on demonstration observed by a certified trainer).
  • Covid vaccination due to job nature and multiple facility requirements.
  • High school diploma or General Equivalency Degree (GED).
  • Ability to read and follow written instructions and document care given.
  • Understanding of hospice philosophy and comfort providing specialized care to the terminally ill.
  • Prior experience in hospice, orderly, acute, rehabilitative, or long-term care facilities; 1 year previous home health/hospice experience preferred.
  • Basic computer skills to document in electronic medical records (EMR).
  • At least 18 years of age.
  • Licensed driver with automobile that is insured in accordance with organization requirements and is in good working order.
Compensation

The estimated annual salary for this position is $$$48,000-$62,000 per year, depending on qualifications and experience.
