Job Description:

Overview

  • Plan and deliver compassionate care to patients utilizing the nursing process of assessment, planning, interventions, implementation, and evaluation.
  • Effectively interact with patients, families, and other interdisciplinary team members while maintaining standards of professional nursing.

Key Responsibilities

  1. Assess and evaluate patient and family needs, including physical, spiritual, and psychosocial aspects.
  2. Develop and implement individualized plans of care in collaboration with patients and families, consistently updated and reviewed to reflect assessment findings.
  3. Review and update plans of care, including interventions, medication profiles, and visit frequencies to meet changing patient needs.
  4. Collaborate with hospice teams in regularly scheduled care conferences, admissions, and Interdisciplinary Team meetings to ensure coordination of services.
  5. Document physicians' orders in electronic health records for accurate information and safe delivery of services.
  6. Supervise, teach, and evaluate care provided by Personal Care Specialists, Volunteers, and other caregivers to ensure compliance with plans of care.
  7. Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions.

Required Skills and Qualifications

  1. Graduate of an accredited nursing school; Associate of Science in Nursing (ASN); Bachelor of Science in Nursing (BSN) preferred.
  2. Active Registered Nurse license in the State of Ohio without any board actions.
  3. Two years of acute care nursing experience preferred.
  4. Certification in Hospice & Palliative Medicine (CHPN) preferred; required within two years of employment.
  5. CPR Certified.
