End of Life Support Specialist Job Description

We are seeking a compassionate and skilled Nursing Care Liaison to join our team at Ohio's Hospice.

About the Role:

This is a driving position supporting the Dayton/Montgomery County and surrounding areas, with flexible schedule hours for days/evening shift hours, including an every 3rd weekend rotation.

Responsibilities:

  • Admit patients into the hospice program efficiently and provide them and their families with the necessary resources at the end of life.
  • Maintain a conversion rate of 75% by utilizing the electronic medical record accurately and effectively.

Requirements:

  • RN's must be graduates of accredited nursing schools, with Associate of Science in Nursing (ASN) or Bachelor of Science in Nursing (BSN) preferred.
  • Active Registered Nurse license in the State of Ohio without any board actions.
  • Two years of acute care nursing experience are preferred.
