Position Overview:

The Patient Care Coordination Registered Nurse will serve as the primary advocate and educator for the management of post-acute care services within our healthcare facilities. This role involves evaluating patients for post-acute care needs, facilitating the clinical transition to home health and hospice services as necessary, and ensuring the appropriate post-acute care environment is established. The Coordinator will cultivate relationships with healthcare providers, therapists, patients, and their families, focusing on collaboration with care management and clinical teams to create effective discharge plans that encompass post-acute services.

Key Responsibilities:

+ Conduct thorough assessments of patients and their families, considering physical, psychosocial, and functional factors that influence care outcomes and suitability for home health and hospice services.

+ Educate patients and families about available post-acute care options, balancing their preferences with the requirements for safe and effective ongoing care.

+ Identify and communicate patient and family needs or concerns to the physician, care management, and clinical teams.

+ Facilitate home health care referrals within designated facilities and perform bedside assessments to determine the appropriateness of home or hospice care admissions, while also informing patients and families about discharge plans and expectations for home care and hospice services.

Qualifications:

+ Associate Degree in Nursing or higher.

+ At least two years of experience in post-acute care settings (e.g., home health, skilled nursing facilities, or hospice) or care management within a healthcare facility.

+ Proven experience in customer service and public interaction.

+ Familiarity with medical terminology and the post-acute referral process.

+ Current Registered Nurse license.

+ Basic Life Support certification.

+ Valid driver's license, automobile insurance, and a safe driving record.
