Role Overview:
The Patient Care Coordination Nurse serves as the primary advocate and educator for managing post-acute care services within AdventHealth facilities. This role involves evaluating patients for post-acute care, facilitating clinical transitions to home health and hospice services as necessary, and ensuring appropriate placement in post-acute settings. The Coordinator will maintain essential relationships with physicians, post-acute providers, therapists, patients, and their families. Assigned to a specific hospital or specialty, this position collaborates with care management, physicians, and clinical teams to formulate discharge plans that necessitate post-acute services across the AdventHealth care continuum.

Value Added to the Team:
+ Conduct thorough post-acute assessments covering physical, psychosocial, and functional dimensions of patients and their families, assessing their impact on potential outcomes to determine suitability for Home Health and Hospice care.
+ Educate patients and families about available post-acute options, balancing their preferences with the requirements for providing safe and effective ongoing care.
+ Identify and communicate patient/family needs or issues to physicians, care management, and the clinical team.
+ Facilitate coordination of home health care referrals within designated hospitals. May perform bedside evaluations to assess the appropriateness of home or hospice care admissions and guide patients/families regarding discharge plans and expectations for home care and hospice services.


Qualifications:
Essential Skills and Experience:
+ Associate Degree in Nursing or higher.
+ At least two years of experience in post-acute care (e.g., home health, skilled nursing facilities, or hospice) or care management within a facility.
+ Strong public interaction skills and exceptional customer service abilities.
+ Familiarity with medical terminology and the post-acute referral process.
+ Registered Nurse license.
+ Basic Life Support certification.
+ Valid driver's license, automobile insurance, and a safe driving record.
