Position Overview:
The Connected Care Coordinator plays a pivotal role as the primary advocate and educator for managing post-acute care services within AdventHealth facilities. This role involves evaluating patients for post-acute care needs, facilitating the clinical transition to home health and hospice services as necessary, and ensuring appropriate placement in post-acute settings. The Coordinator is tasked with nurturing relationships with healthcare providers, therapists, patients, and their families. Assigned to a specific hospital or specialty, this position requires collaboration with care management, physicians, and clinical teams to formulate discharge plans that encompass post-acute services across the AdventHealth care continuum.

Key Responsibilities:
+ Conduct thorough assessments of patients and their families, considering physical, psychosocial, and functional factors that may influence care outcomes and appropriateness for Home Health and Hospice services.
+ Educate patients and families about available post-acute care options, balancing their preferences with the necessity for safe and effective ongoing care.
+ Identify and communicate patient and family needs or concerns to the physician, care management, and clinical teams.
+ Assist in coordinating home health care referrals within designated facilities, including bedside assessments to evaluate the suitability for home or hospice care admissions, while informing patients and families about discharge plans and service expectations.


Qualifications:
Required Skills and Experience:
+ Associate Degree in Nursing or higher.
+ A minimum of two years of experience in post-acute care (e.g., home health, skilled nursing facilities, or hospice) or care management within a healthcare setting.
+ Proven experience in public interaction and exceptional customer service abilities.
+ Familiarity with medical terminology and the post-acute referral process.
+ Active Registered Nurse license.
+ Basic Life Support certification.
+ Valid driver's license, automobile insurance, and a safe driving record.
