Descripción del Trabajo:
- Actúa como Especialista en Apoyo Comunitario para la Instalación.
- Coordina las altas de los pacientes de acuerdo con las políticas establecidas y los requisitos de licencias estatales y federales.
- Supervisa para asegurar que se completen las secciones apropiadas del MDS según lo asignado.
- Asegura que se cumpla un programa social significativo adecuado a las necesidades de cada paciente.
- Refiere a todos los residentes o familias a la agencia o recurso apropiado si el servicio no puede ser proporcionado dentro de la Instalación.
- Colabora estrechamente con los Directores de Unidad/Supervisores de RN y miembros de la familia o residentes para obtener información sobre admisiones, altas y cambios de habitación.
- Asiste con las admisiones de nuevos residentes, completando la documentación relacionada y facilitando la adaptación al nuevo entorno.
- Ayuda a las familias o residentes con la colocación y la información de alta según sea necesario.
Requisitos del Trabajo:
- Título universitario de cuatro años y un año de experiencia en el campo de la salud o una combinación equivalente de educación y experiencia.
- Conocimiento de las regulaciones locales, estatales y federales en relación con las necesidades sociales de los pacientes.
- Experiencia en planificación de admisiones y/o altas preferida.
Realizamos verificaciones de antecedentes y mantenemos un lugar de trabajo libre de drogas.
