Clinical Care Coordinator

Position Overview
The Clinical Care Coordinator is responsible for overseeing the delivery of quality and economically viable health and social services for patients with complex medical needs. This role involves evaluating patients' clinical requirements and ensuring they qualify for admission and ongoing care. Effective communication is essential for coordinating services with the interdisciplinary care team, patients, their families, and insurance providers. The Clinical Care Coordinator also initiates and facilitates follow-up services post-hospitalization, which may encompass physician appointments, home health services, hospice care, medical supplies, outpatient treatments, and more. In cases where alternative placements are necessary, the Coordinator manages the referral, approval, and placement procedures.
Key Responsibilities
- Conduct timely Utilization Reviews in alignment with the established Utilization Review plan.
- Perform initial assessments upon patient admission and provide regular updates to families.
- Organize and lead weekly interdisciplinary team meetings.
- Oversee the discharge planning process for all patients.
- Act as the Clinical Care Coordinator representative on various committees, including the Utilization Review Committee, Clinical Practice Team, and others.
- Supervise non-RN Case Managers.
- Perform additional duties as assigned.
- Associate's degree in Nursing is required.
- Bachelor's Degree in Nursing is preferred.
All certifications must be obtained upon hire unless otherwise specified.
- Current Basic Life Support (BLS) certification from the American Heart Association is required.
- Must be licensed to practice as a Professional Registered Nurse in the State of Georgia or possess an active Multi-State Registered Nurse Compact License.
- CCM certification is preferred.
Must hold a current Registered Nurse license in Georgia or have the privilege to practice under the Enhanced Nurse Licensure Compact (eNLC). A valid BLS card from the American Heart Association is required on the first day of employment.
Experience Requirements
- A minimum of 2 years of nursing experience is required.
- Preferred experience includes 2 years in a Rehabilitation or Long-Term Acute Care Hospital with case management responsibilities.
Candidates should demonstrate excellent verbal and written communication abilities, strong relationship-building skills, and a comprehensive understanding of the healthcare environment. Leadership qualities are essential, including the ability to implement and manage change effectively, inspire and coach others, and model professional practice. A commitment to continuous learning and an understanding of healthcare financing, strategy, and operations are crucial. Candidates should also be adept at utilizing data and technology to support decision-making processes. Furthermore, knowledge of risk management and adherence to various accreditation and regulatory standards is vital for advancing industry benchmarks through effective management and outcome measurement.
