Position Overview

The Licensed Practical Nurse (LPN) operates under the supervision of a Registered Nurse or Nurse Manager, adhering to the LPN Scope of Practice. This role involves delivering comprehensive care to patients, focusing on their physical, emotional, developmental, psychosocial, and educational needs. The LPN is tasked with ensuring the provision of high-quality care, which includes preparing patients for examinations, assisting in treatments, coordinating diagnostic tests, and documenting care in medical records. The LPN also plays a vital role in educating patients and their families. Participation in staff meetings, training sessions, special projects, and ongoing education is essential. Additionally, the LPN may act as a mentor for newly hired colleagues.

Key Responsibilities

DIRECT PATIENT CARE

  • Evaluates and addresses the comprehensive needs of patients in accordance with physician directives, under RN supervision.
  • Conducts routine nursing tasks to fulfill patient requirements while under RN oversight.
  • Adheres to established productivity benchmarks.
  • Exhibits proficiency in accessing and de-accessing ports, including flushing lines and maintaining sterile techniques.
  • Demonstrates expertise in medication administration as per organizational guidelines.
  • Seeks guidance, training, or supervision from leadership as necessary.
  • May assign tasks to unlicensed personnel, as appropriate and within the scope of practice.
  • Delivers patient-centered care that is tailored to the specific characteristics of the population served (e.g., age, gender, health conditions) in a culturally sensitive manner.
  • Collaborates with patients, their families, and significant others to create a safe and comfortable environment for care delivery, ensuring that patient goals are prioritized in the care plan.

COORDINATION OF CARE

  • Facilitates care coordination with the RN and other healthcare team members.
  • Communicates changes in patient conditions to the RN, physician, or clinical supervisor as appropriate.
  • Works alongside the RN to develop and adjust individualized care plans based on patient needs and physician orders.

PATIENT EDUCATION

  • Acts as an educator for patients and caregivers to promote self-care and support.
  • Utilizes teach-back methods to ensure understanding of self-care instructions by patients and/or caregivers.

PROFESSIONAL DEVELOPMENT

  • Engages in the orientation and training of new staff and nursing students as appropriate.
  • Responsible for attending required continuing education and training sessions.
  • Maintains current knowledge in clinical practices related to oncology patient care.

DOCUMENTATION AND COMMUNICATION

  • Ensures accurate and timely completion of required documentation.
  • Demonstrates understanding and application of regulatory standards.
  • Responsible for managing communications, including returning patient calls and checking work email regularly.

Qualifications

  • Practical Nursing Diploma is required.
  • Licensure as a Practical Nurse is mandatory.
  • Basic Life Support (BLS) certification is required upon hire.
  • Previous experience in healthcare, particularly in oncology or outpatient settings, is preferred.
  • Strong leadership, problem-solving, organizational, and time management skills are essential.
  • Excellent assessment and clinical skills are required.
  • Strong written and verbal communication abilities are necessary.
