Position Overview

The Inpatient Heart Failure Clinical Navigator is a licensed professional nurse with specialized clinical knowledge who operates under the guidance of the primary Heart Failure Destination Unit Nurse Manager. Key responsibilities encompass the creation, execution, and upkeep of the heart failure program, which includes evaluating and managing heart failure patients, formulating clinical practice guidelines to enhance patient outcomes, gathering and analyzing heart failure data, ensuring adherence to heart failure protocols and program initiatives, driving performance enhancement, educating staff, and maintaining compliance with regulatory standards. The RN Heart Failure Navigator acts as a clinical mentor, educator, and liaison for the heart failure team, hospital personnel, and the community, collaborating with the medical director and nursing leadership to foster improved patient outcomes.

Core Responsibilities and Essential Functions

Patient Care
  • Guides heart failure patients throughout their hospital stay from admission to discharge.
  • Collaborates with outpatient teams to facilitate a smooth transition across the continuum of care.
  • Implements and coordinates heart failure treatment in conjunction with team members.
  • Provides heart failure-specific education to patients, family members, and caregivers.
  • Exhibits advanced knowledge and practice in assessing, diagnosing, treating, and evaluating human responses to actual and/or potential health issues through role modeling and consultation.
  • Engages with healthcare providers, ancillary services, patients, and significant others to assess and identify individual patient care needs.
  • Promotes the delivery of safe patient care and optimal outcomes. Clinical Expert/Resource
  • Acts as a clinical expert and resource for hospital staff and other healthcare professionals within the organization and community.
  • Monitors and evaluates heart failure data to collaborate with the heart failure team in identifying and participating in performance improvement initiatives.
  • Ensures adherence to clinical guidelines, tracks deviations from guidelines/care paths, and reports outcome data to cardiac leadership.
  • Maintains compliance with Joint Commission and regulatory standards to uphold certification; ensures timely completion of all required reports submitted to the Joint Commission.
  • Regularly meets with the System Heart Failure Program Manager to ensure alignment and cohesion of the program. Professional Growth and Development

  • Integrates appropriate teaching and learning principles into the design and delivery of educational programs and materials.

  • Provides support and consultation for disease-specific community and staff education.

Required Minimum Education

Bachelor's Degree in Nursing Required and

Graduated from an accredited nursing program Required and

Required Minimum Experience

Minimum of 2 years recent clinical experience in cardiology with demonstrated teaching experience in clinical and community settings Required
