**Job Summary**

GeorgiaGov is seeking a highly motivated and compassionate Support Services Coordinator to join our team. As a key member of our support staff, you will play a vital role in ensuring the well-being and success of our clients.

**Key Responsibilities:**

  • Develop and Implement Service Plans: Collaborate with clients, families, and healthcare professionals to create and implement personalized service plans that meet the unique needs and goals of each individual.
  • Provide Life Skills Training: Teach and support clients in developing essential life skills, including basic personal and social skills, simple academics, and work skills.
  • Monitor and Document Client Progress: Regularly observe and document client behavior, progress, and any changes in their condition, ensuring accurate and timely reporting to the direct care treatment team.
  • Arrange for Medical Care and Support: Coordinate medical appointments and provide support to ensure clients receive necessary medical care and services.
  • Support Social and Community Integration: Facilitate opportunities for clients to build relationships and social networks, participate in community activities, and engage in recreational pursuits.
  • Provide Transportation and Accompaniment: Safely transport clients to appointments, outings, and other activities as needed.
  • Counsel and Advise Clients: Offer guidance and support to clients in resolving daily challenges, making informed decisions, and addressing behavioral issues.
  • Participate in Team Meetings and Case Reviews: Collaborate with the direct care treatment team to evaluate client needs, plan for continuing services, and ensure seamless care coordination.
  • Perform Household Duties: Assist with household tasks, including cleaning, laundry, and meal preparation, to maintain a safe and comfortable living environment.

**Requirements:**

  • High School Diploma or Equivalent: A high school diploma or equivalent is required, along with six months of relevant job-related training or experience.

**Work Environment:**

This is a full-time position working in a residential setting. The ideal candidate will possess excellent communication and interpersonal skills, be able to work independently and as part of a team, and demonstrate a genuine commitment to supporting individuals with diverse needs.
