We are seeking a skilled Client Relations Support Associate to join our team at CommonSpirit Health at Home. As a key member of our organization, you will play a vital role in forging influential relationships with key customer sources, including long-term care facilities.

Responsibilities:
  • Manage client connections with utmost professionalism and efficiency
  • Ensure accurate client information and files
  • Deliver unparalleled customer satisfaction through timely and accurate responses to customer and patient questions and concerns
  • Explain private pay, Medicare, and Medicaid guidelines, insurance plan benefits, and organizational policies to patients and caregivers
  • Coordinate services for patients and caregivers, ensuring smooth transitions to home or assisted living environments

Qualifications:
