Occupational Therapist for Home Health Visits

We are seeking an experienced Occupational Therapist to join our Home Health Team. As an Occupational Therapist, you will be responsible for providing high-quality home health visits to our patients in the comfort of their own homes.
You will work closely with our interdisciplinary team to develop and implement individualized plans of care that promote independence, safety, and overall well-being for our patients.
Key Responsibilities:
- Evaluate patients' functional status and develop treatment plans to improve their ability to perform daily activities.
- Conduct home assessments to identify potential hazards and barriers to independent living.
- Collaborate with patients, families, and healthcare professionals to ensure coordinated and comprehensive care.
- Develop and implement adaptive equipment and splinting programs to enhance patients' functional abilities.
- Provide education and training to patients and their families on proper use of adaptive equipment and techniques.
Requirements:
- Graduate of an occupational therapy curriculum accredited by the American Occupational Therapy Association.
- Registered with the National Registration Examination of the American Occupational Therapy Association.
- Two years of clinical experience as an occupational therapist.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
